Holiday Assistance Program 2007

GENERAL NEEDS:  Groceries, household cleansers, laundry supplies, paper products, hygiene products, underwear, clothing (sizes given), puzzle books, large print reading materials, large print Bibles, Refresh eye drops, Tums or Phoslo, Tylenol products, Fiber laxatives, Ensure or Boost (all flavors), Bladder-control briefs (large to XL), space heaters, blankets or afghans for dialysis.  Most patients like sweatsuits to wear to treatment.  Coats, scarves, and gloves are always appreciated.

Many of our patients enjoy coloring books, crayons or markers.  The women also like stuffed animals and romance novels.  Men like wallets, western novels, sports magazines.  All can use help with utility and medicine bills.

SPECIFIC CLIENTS:

PA -Female dialysis patient.  Diabetic, Asthma, Gout

Daughter, age 14 - med shirt, 11/12 pants, 6.5 shoe - Books, music, scrabble, jewelry, gift card.

Son, age 15 - 4XL shirt, 36 pants, 12.5 shoe, game cube and games, gift card

Daughter, age 16 - 2XL shirt, 16 pants, 9 shoe, CD player w/radio, gift card, jewelry.

Patient- 26/28 shirt and pants, 12W shoe, word search, night gowns, housecoat.

Family could also use groceries, large print bible, cleaning supplies, paper products, twin linens, Ensure or Boost (any flavor), blanket or afghan for dialysis.

BA - Female dialysis patient. Visually impaired, Diabetic, Large print Bible, magnifying glass, 3XL shirt and pants, shoes 8.5 wide, gowns and robes, slippers, lounging dress (caftan), coat 4XL, cap, gloves, coloring books and crayons or markers, paint by number kit, embroidery and puzzle books.

JB - Male dialysis patient.  Lives with spouse.  Only requested Boost-chocolate or strawberry.

AB - Female dialysis patient. Coat 2XL, Shirt 2XL, pants 16, shoes 10 wide, loves animals, nature, wildlife and fishing, king size linens, Ensure or Boost, vanilla, large print books especially Bible.

RC - Female dialysis patient.  Pants 18 waist, 2XL or 18 shirt; enjoys bead crafts and other crafts, puzzle books. Daughter, age 15  Family needs bladder control briefs, large, white socks, large print books and Bible, Diabetic food items, Chocolate Boost or Ensure.

ED - Female dialysis patient.  Coat 2XL, queen linens, pants 22 waist, shoes 9. Blanket for dialysis.  Enjoys bingo, reading, driving, and getting away for weekends.

PF- Female transplant patient and husband.  Pt- XL or 16 pants (elastic waist); shores 8.5.  Husband- 40W/30L pants, shoes 12D, dress shirt 18, pullover shirt, XL.  They enjoy reading the Bible, bowling cooking and singing. 

AH - Female dialysis patient. Shirt 6XL, pants 4XL, coat 5XL, shoes 13.  Utility asst, white socks, groceries.

DH - Male dialysis patient with wife.  Likes chocolate Boost and remote cars.  2XL shirt, pants 44-30, shoes 11, coat 3XL.  Wife- 3XL shirt, pants 26, shoes 12.  Groceries, Diabetic food items.

EL - Female dialysis patient.  Diabetic; Groceries, Vanilla Boost or Ensure.

SM- Male dialysis patient. Diabetic.  Groceries, Cleansers, White socks, full/queen linens, comic books, utility assistance.

BO - Female dialysis patient.  Groceries.

GR - Male dialysis patient. Groceries, large print Bible, paper products, twin linens, blankets.

JR - Male Dialysis Patient, amputee, lives with wife-sole caregiver.  Strawberry Ensure, XL bladder control briefs, latex gloves-large, bed liners or underpads, help with bills, groceries, gasoline money.

Patient- Shirt and pants, 2XL-likes sweatsuits, gray white or red.  Wife only listed underwear.  (They have been having to skip some bills to pay for medical supplies).

DR - Male dialysis patient, lives with wife.  shirt XL, pants 33/33, shoes 10.5,  needs sweater or jacket, large print Bible and books, queen linens.  Wife - 11 dress, shoes 10.5

TSA - Male dialysis patient, Diabetic.  White socks, large shirt, 38W/32L pants, 12 shoes, XL coat. Loves pickles.

LS - Female dialysis patient and husband.  Diabetic.  White socks, shirt and pants, 18, groceries, vanilla or strawberry boost or ensure, blankets, tums, baby oil.

EMS - Female dialysis patient.  Groceries, queen linens, large print books, magazines, Bible, large shirt, 2XL pants, 10 shoes.

GS- Female dialysis patient, age 79 and husband, age 78.  Large print books and Bible, cleaning supplies, diabetic items, groceries, queen linens, vanilla ensure, blankets, medium shirt and pants, 8.5 shoes, puzzle books, medium coat.  Husband - large shirt, XL pants, 9 shoes, large coat.

TT - Female dialysis patient and husband.  white socks, slippers, large print Bible, groceries, utility asst, twin linens, blankets.  likes word finds and crossword puzzles, medium shirt and pants, 9 shoes.  Husband wears large shirt, medium pants. 

MT - Female dialysis patient, wheelchair bound, groceries, large coat (raincoat with hood pref), blanket for dialysis.
